This form constitutes a legal document. By completing and submitting it, you are booking our services

and agree to be bound by our terms, including any applicable cancellation and rescheduling fees.

All cancelations have a 50% fee of total service.

There is a four hour minimum for each Pour Masters team member for each event.

Payment is due prior to the event date.
